Documents for the camp

 

THE FOLLOWING DOCUMENTS ARE NEEDED FOR YOUR CHILD TO PARTICIPATE IN THE CAMP:

1. Agreement

2. Camp internal regulations (Annex to the Agreement);
3. A copy of the child's birth certificate;

4. Negative COVID-19 test result. RNS test must be made not earlier than 48 h before camp
5. Medical certificate of the family doctor (not older than 7 days from the start date of the camp) with the following information:

  • permission to attend a children's camp with indications of chronic diseases and all kinds of allergies (including medicines);

  • use of special medications;

  • a record of no contact with infectious diseases;

  • information on the vaccine against tick-borne encephalitis;

  • information regarding the vaccinations performed (a copy of the vaccination passport or an entry in the certificate);

  • a statement from a family doctor that the child has no signs of infectious diseases;

  • a statement from the family doctor that the child has not been in contact with Covid-19 patients or their contacts.
